At the request of my patients, I developed this exciting facial ageing solution that is the culmination of 10-years’ work, I call it the Vertical Face Restore-7, which entails 7-steps and helps my patients look at least 7 years younger.
De Silva Vertical Face Restore 7 is a “Turnkey” solution to facial ageing, customised to each individual, these are the 7 Key Steps:

As you age, your face slowly droops with gravity from a ‘V-shaped’ jaw line that is youthful, to a ‘Squarer-shaped’ jaw line. This change occurs as the soft tissues in the face (also known as the “SMAS”) loosed, droop and descend (shown with green lines on the photo below). This leaves the face looking a different shape than it did during your youth, and with the facial ligaments (that are fixed) are responsible for that visible changes seen with facial ageing (shown with yellow arrows below). This shift in shape is the key problem with facial aging, and addressing it is the most important and impactful way to create satisfying results.
Non-surgical solutions that add volume i.e. fillers to the jaw line to reduce jowls often make the jaw line smoother, however they do not make you look more youthful, fillers do not restore the youthful ‘V-shaped’ jaw line. A key component in creating the Vertical Face Restore 7 was a solution to the underlying problem.

Compared with a traditional facelift, the VFR technique offers far more differences than it does similarities, the lift is vertically oriented, not horizontally, which is a major reason for the natural look. A traditional facelift only addresses the jawline by pulling it toward the ears.

Vertical Restore releases the principal facial ligaments in the face and neck, thus allowing the tissue to be elevated and repositioned without pulling against those ligaments. This is the reason the effect of the VFR is so effective and is why, despite the large improvements, the results look fake nor “pulled. “We believe the Vertical Face Restore incorporates a deep-plane facelift approach, offering the most advanced facelifting techniques for the face and neck.
A small proportion of patients benefit from only a facelift procedure, in most patients the neck needs to be improved at the same time as the face to give the best results. When the face drops with gravity, the neck also sags as the neck muscle is linked in terms of facial anatomy, the vertical face restore technique lifts the face upwards restoring the contours and youthful shape of the neck at the same time. When combined with release of ligaments and tightening of the lose neck platysma muscle, the neck is sharper and more youthful in shape. A typical horizontal lift won’t address the neck in the same way as because it pulls the face sideways, key to facial harmony is lifting vertically upwards to restore naturally. The lose skin is then removed behind the ears.
This patient had my Vertical Face Restore-7 technique including a deep plane facelift with lifting of the dropped face in an upward direction restoring natural ‘V-shaped’ jaw line. This deep plane technique requires key release of the facial ligaments to enable a longer lasting facelift technique.

This patient had my Vertical Face Restore-7 technique, a deep plane facelift with reduction of buccal fat pad, enhancing the ‘V-shaped’ jaw line. This technique avoids extra scars inside the mouth by using detailed knowledge of the anatomy, Dr De Silva has pioneered this technique and presented to other surgeons in the UK and internationally.
This patient of Dr De Silva had a small chin that resulted in loss of jawline definition (shown with green lines on the photo below).. Dr De Silva inserted a large chin implant, a customised chin implant specifically for this patient’s jawline to enhance the jaw line and neck definition while maintaining a natural V-shaped jaw line. A key step is sculpting the implant to match the jawline, without this it will result in an older more masculine appearance.

Preservation of the natural anatomy is of key importance with natural looking blepharoplasty results. Too much removal of skin and soft tissues results in a different looking person. This can be seen in many celebrities, some years ago Renee Zellweger underwent eyelid surgery, afterwards she looked different, with good natural eyelid surgery it is likely that most people will not notice that you have undergone surgery.
Often the tail of the eyebrow descends with gravity and the best results, lifting with temporal (lateral) brow lift enhances the appearance in this area, I like to hide these scars completely within the natural hair (many patients ask if this requires shaving of my hair, no this is never required!). This patient had my Vertical Face Restore-7 technique including a temporal (aka lateral) brow lift and blepharoplasty to rejuvenate her tired eye appearance. The key steps are lifting and use of fat transfer to give a more youthful appearance. Older techniques involved removing soft tissues this thins the natural anatomy and looks more tired long-term.

The area around the lips is often a challenging area and is unique to each individual person. This area requires a cautious approach to conceal scars in natural creases under the nose. The deep lines require release of the deep facial ligaments, vertical lifting often in combination with fat transfer for restoring natural volume.

This patient of Dr De Silva had my Vertical Face Restore-7 technique including a Lumenis laser resurfacing to rejuvenate the skin. Although there are many types of laser for more than 10years the Lumenis has emerged as the most powerful laser device able to reduce fine lines, pore size and age spots at both superficial and deep levels of the skin. I use in combination with regenerative medicine techniques to enhance recovery of the skin.

This patient of Dr De Silva had my Vertical Face Restore-7 technique in a 7.5-hour procedure with Sedation (‘Twilight’) Anaesthesia. Although the procedure took hours to complete, she was able to walk out of the operating theatre ONLY 30 minutes after the procedure. The equivalent recovery with general anaesthesia is hours and all patients have more recovery and potential side effects. Deep plane facelift surgery lasts for up to 10 years and maintenance of your appearance after surgery will enhance longevity of results. Although with surgery we can address centimetres of facial ageing, non-surgical treatments can enhance millimetres.
