The Perfect Jaw

Fillers: How Much Is Too Much

Dark circles under the eyes, deep “smile lines,” droopy nose, and sunken cheeks are some of the most apparent signs of aging that appear on the face as we get older. However, in recent years, technological advancements in the cosmetic field have allowed millions of Americans every year to flock to cosmetic surgeons in order to turn back time and restore their youthfulness. According to the American Society of Plastic Surgeons (ASPS), a total of 15.6 million cosmetic procedures, both surgical and non-surgical, were performed last year. A large percentage of these procedures included the use of injectable fillers.

Fillers are substances which can be injected directly through the skin using a syringe. They are very versatile and can be used for various purposes. Some common uses include fixing scars, filling up wrinkles and creases, enhancing cheeks and lips structure, and improving the contours of the face.

Two of the most common types of fillers include hyaluronic acid (used in Restylane and Juvéderm) and purified botulinum toxin A (used in Botox and Dysport).

  • Restylane: This filler is most effective for increasing the plumpness of the face and bringing more volume to the skin. Deep wrinkles and skin folds can be easily reduced by Restylane fillers. On average, this filler will maintain its effects for six months and may even last for up to nine months.
  • Juvéderm: Another popular choice of filler is Juvéderm. This works great for lip augmentation, reducing lines around the lips, treating acne scars, and minimizing “smile lines.” If done properly, Juvéderm should last for six months.
  • Voluma: A newer generation of filler that has the advantage of a cross-linking structure, this enables the filler to last for considerably longer, 1-year or more. This filler works well for volume augmentation in specific parts of the face.
  • Botox: As the most well-known filler available for cosmetic procedures, Botox is the ideal choice for you if you are looking to get rid of crow’s feet, wrinkles on the forehead, frown lines, or to treat muscular spasms on the face. Typically, Botox will maintain its results anywhere from three to four months.
  • Dysport: Dysport is a relatively new filler which is very similar to Botox. It relaxes muscles that cause wrinkles on the face. Thus, it is a good option for treating wrinkles around the eyes, lips and on the forehead. When compared to Botox, Dysport is less expensive, as well.

Regardless of which injectable filler you use, inserting too much filler or improper treatment can cause severe side effects, such as malformed and irregular features, excess swelling and bruises, or weakened muscles. Therefore, it is crucial that you only have an experienced surgeon perform the procedures to ensure your safety and to achieve impeccable results.

Do Non-Surgical Necklifts Really Work?

Increasingly, individuals seeking to rejuvenate their appearance have been inquiring about minimally invasive rejuvenation therapies, a set of techniques collectively known as a “non-surgical facelift.” Lauded as a way to reap the benefits of cosmetic work without having to endure the recovery time associated with surgery, the non-surgical facelift is an option for those who want a visible boost, but are not ready for a more extensive procedure.


What Is A Non-Surgical Facelift?

A non-surgical facelift can include volume enhancers, smoothing, resurfacing and tightening treatments. To combat volume loss and skin laxity, dermal fillers, such as Juvaderm, Sculptra, Restylane and Radiesse, are commonly injected into sunken areas of the face–often the cheeks–to restore youthful plumpness. With no incisions and sutures involved in the procedures, there is no drawn out recovery time or need for post-surgical pain management. Dr. De Silva utilizes non-needle techniques with the latest technology to minimize the chance of bruising from these treatments compared to traditional treatments. Without bruising most patients there can be no signs a patient has undergone treatment.

So how does the non-surgical facelift stack up against a traditional facelift? Both approaches have their pros and cons. Non-surgical facelift treatments can be highly effective on skin that shows early signs of aging. Therefore, the best candidate is generally younger and has skin that still has some elasticity. A non-surgical facelift cannot counteract note able skin laxity and more advanced signs of aging. While the results of a non-surgical facelift can be long-lasting, they will not be as enduring as a traditional surgical facelift. Typically non-surgical techniques can last from 6 months to over a year, depending on the type of treatment. Though a surgical procedure involves more time and carries some risks, the positive outcome of a facelift can last for many years.

While a non-surgical facelift will not address severe sagging in the face and neck and jowls, the techniques can be transformative on patients who younger, particularly those in their 30s and 40s. For those who would like to avoid surgery and downtime, the non-surgical facelift is an excellent option. Not all the treatments used in non-surgical facelifts are appropriate for every individual, so it is essential to have a comprehensive consultation so that the results can be optimized.

More Celebrity Plastic Surgery or Strategic Makeup?

Uma Thurman’s recent red carpet appearance drew a lot of attention from the press for her “drastic change in appearance.” Many media outlets have speculated on plastic surgery she may have undergone, recalling the plastic surgery speculation furor that happened after Renee Zellweger’s recent changed appearance as well. Though Uma’s new look is very different from her previous red carpet styling, these differences are more due to makeup and styling than plastic surgery. Her eyebrows have been styled much straighter and fuller than before, which makes her look younger. Her hair is slicked back, which allows her features to stand out more than her previous loose face framing locks. Her makeup artist also used more contouring techniques to highlight her cheekbones.

Through these cosmetic changes, we can still see that the fundamental facial structure has stayed consistent. Her eyelid spacing has not changed, which was one of the big indicators with Renee Zellweger’s rumored blepharoplasty.  Uma’s red lips may draw more attention, but they are still the same size and shape as before, and her nose has stayed the same as well. Liquid Facelift vs. Traditional Facelift- Which One Should You Get?

One of the main differences between the two options is that a liquid facelift is a non-surgical alternative to the traditional facelift procedure. While a traditional facelift requires an incision, the use of either general or local anesthesia, and involves a more intensive recovery period, the results of a liquid facelift are nearly immediate and require no downtime. A liquid facelift, as performed in our office, is a broad term that generally includes injectable fillers, like wrinkle reducing treatments and Juvederm. The term can also include the PRP facelift, a treatment in which platelets from your own blood are extracted and transported to the face in order to revitalize and refresh your complexion. A liquid facelift will leave no signs of scarring, can visibly restore lost facial volume, and is a good option for those who are hoping to achieve a younger, more refreshed look but who are afraid of or otherwise unwilling to undergo surgery. Whereas a traditional facelift can take up to two hours to perform and requires you to take about a week off from work, your liquid facelift will be complete in 15 minutes. You will be in and out of the office without having to sacrifice your daily routine to devote to a proper recovery and can literally have the treatment done during a lunch break from work.


Still, this shorter process is directly reflected in the results that the respective procedures yield. Perhaps the most important difference when evaluating the liquid vs. traditional facelift is that the results of the traditional facelift are more dramatic and long-lasting. A traditional facelift has the ability to make you look up to 15 years younger, and is generally sought after by relatively older patients whose skin has loosened as a result of aging, exposure to the sun, or the body’s natural decrease in the skin’s collagen supply. During a traditional facelift, Dr. De Silva will essentially “lift” the saggy skin that has accumulated around your face. This approach is meant as a solution for a more pronounced condition, where the patient demonstrates considerable skin laxity and looseness. As expected, a surgery will yield much more dramatic results than that of a liquid facelift and since underlying muscles are put back to their original location and excess skin is removed, patients will look younger than their age even 10 or 20 years after their facelift surgery. Traditional facelift is recommended for patients who are between the ages of 45-70, while the liquid facelift is more popular among younger patients who are not experiencing as much wrinkling, sagging, and skin laxity.
