There are different techniques that can be used to narrow the nostrils, broadly speaking these can be divided into those with scars that are beneath the nostrils and those on the side of the nostrils. I used some of the cartilage from her septum as graft material to make her nose straighter and improve her breathing, termed spreader grafts. There was notable asymmetry of her bones and she needed osteotomies to improve this. Piezo ultrasound technology can be used to reduce the bones a controlled way. With her ears there were two parts to improving the shape of ears, reducing the volume of cartilage in the ear and enhancing a crease at the top of ear to tuck the ears in. Your ears are made out of cartilage with a memory so they want to go back to their original shape, care is required to ensure they stay as they are in their new position. I completed a natural rhinoplasty with septoplasty under sedation anaesthesia (twilight anaesthesia). I utilised specialised cartilage techniques to refine and reshape the cartilages narrowing both the bridge of his nose and nostrils, piezo ultrasound can be used for boney reshaping and narrowing, although not suitable for all patients. This patient had oilier skin which can sometimes be oilier for a few months after surgery before returning to normal. I prefer to make any scars relatively discrete on the underside of the nose, hiding a scar in the natural shadows of the nose. She had wanted a smaller and more feminine natural looking nose. I spent time with her to discuss expectations from surgery, particularly with thick ethnic skin, can result in considerable swelling and patience for the final result. I use anti-swelling medicine in the recovery period to speed up recovery in these circumstances. Ethnic rhinoplasty requires different techniques to caucasion rhinoplasty as the anatomy is subtly different. Many of my patients are often worried about looking too different after surgery, however with good surgery, despite quite a difference, the great majority of the time people do NOT notice! The best complement is a friend said I looked good but they could not put their finger on why, and then it is up to you what you want to say!
