When she met with Dr De Silva, they discussed her concerns and he suggested that upper and lower eyelid surgery would best address her concerns. Upper eyelid surgery removed excess skin, fat and a small amount of muscle above the eye (Upper Blepharoplasty), clearing away the skin that makes the eyes look tired and older. Lower eyelid surgery repositioned some fat to areas of hollowing in the eyelid through a transconjunctival incision (Hidden incision on the inside of the eyelid with no scarring) and addresses puffiness below the eyes (Lower Blepharoplasty). She also went underwent eyelid support with strengthening of the eyelid tendon (Canthopexy) and TCA skin resurfacing (TriChloroAcetic acid peel of the lower eyelid skin). As a combination procedure, upper and lower eyelid surgery with skin rejuvenation work together to refresh the eyes and make the eyes appear much more youthful.

In standard upper eyelid surgery, fatty deposits (which make the eyes appear puffy), excess skin and muscle are reduced and redistributed to smooth and tighten the area. After this woman’s surgery, her eyes appeared fuller, smoother and more noticeable to others, as can be seen in the “after” picture.
The “after” picture was taken about 6-months after the surgery. They show a decrease in the sagging above the eyes, making her eyes seem larger and more alert than in the “before” pictures. Important considerations in this person include some fat should be transposed and preserved to avoid hollowness, conservative surgery is advised to give patients natural looking results. The ‘after’ photograph shows the appearance of the eyelids at 6-weeks after surgery and shows an absence of fat bags and a smoothened contour between his eyelid and cheek associated with youth. There are no incision in the skin and no clinical signs of surgery resulting in a completely natural looking result. The patient was very happy with the resulting appearance.

She was told this could only be improved with a further more invasive brow lift surgery. She was reluctant to have this surgery and went to see Dr De Silva a facial cosmetic and plastic surgeon based in London for a further opinion. Dr De Silva advised her the appearance could be improved with a revision blepharoplasty and under local anaesthesia, although most of the incision could be hidden in the natural skin creases, a small 8mm part of the incision was hidden at the corner of the eye in the area where she had commonly occurring lines also know as crows feet. She underwent Revision facial plastic surgery under that gave her the result that she had always wanted. After surgery numerous people commented on how she looked rejuvenated, though no one could tell she had undergone surgery.

However the surgery had been a disappointment and had not given the rejuvenation she had hoped and she had not been happy with the results. After the surgery there was asymmetry with multiple folds of skin and lines around the upper eyelids and a more tired and older appearance. She went to see Dr De Silva a facial cosmetic and plastic surgeon based in London for revision eyelid surgery. Dr De Silva found on assessment that she eyelid scars that were 50% higher than normal, making them visible on her face and creating un-natural multiple skin creases her upper eyelid. According to Dr De Silva “Multiple skin creases and lines in the upper eyelids is an appearance that is associated with facial ageing and when asymmetrical is very noticeable. Having multiple lines can make a younger patient look older and is an avoidable with meticulous attention to detail.
