With today’s work and social environments, there is an increasing desire to look refreshed and less tired, to boost self-confidence and reflect your personality.
With cosmetic procedures for men rising in popularity every year, more male patients are looking to refresh their appearance with male facelift and neck lift procedures. Patients want to look completely natural, reversing the signs of ageing that give men a tired appearance – loose skin and bands in the neck, loss of smooth jawline with increased lines, and wrinkles.
Dr Julian De Silva is a facial cosmetic and plastic surgeon in London. His approach to facelift and neck lift surgery delivers a natural, artistic result by focusing on the individual characteristics of each patient.

One of the earliest signs of facial ageing is changes and sagging of the lower third of the face, the presence of jowls (the prominent soft tissue at the jawline) and a sagging neck with loss of the natural curvature and defined neck angle.
Along with eyelid changes, the neck is one of the first signs of facial ageing. A facelift for men that usually includes a neck lift restores the natural contours of the jaw and smoothens the angle of the neck beneath the jaw.
A rejuvenation resulting from a natural neck angle can turn back the years and make a person look years younger.
When considering a male facelift, Dr De Silva believes it is essential to be conservative, focusing on improving the neck and jawline while preserving some of the natural facial lines.
Overpulled or over-lifted appearances give a ‘done’ look that is key to avoid.
Over-lifting in men’s faces can result in the feminisation of the face, and Dr De Silva is focused on conservative natural-looking results and takes measures to avoid this.
The differences between male and female facial anatomy must also be considered:
Facelift surgery is a surgery of finesse and detail.
Scars are very important features of facial surgery, particularly in men, where shorter hair means that scars are potentially more visible.
Unsatisfactory scars are caused by a combination of factors including surgical technique, the position of scars, closure techniques (stitches vs staples) and genetics (1% of patients can have healing issues with scars secondary to their genetics).
Taking meticulous measures to minimised and hide scars are key to success:

Face and neck lift surgery is one of the most common plastic surgery procedures Dr De Silva completes for men on a daily basis.
Dr De Silva utilises a spectrum of different male facelift and neck lift techniques from Los Angeles and New York. Every person differs in their facial shape, size, symmetry, ethnicity, genetic and exposure to environmental elements such as sun and smoke.
As a consequence, no two faces are the same. Dr De Silva tailors his surgical technique in accordance with multiple patient factors.

In order to give his patients the most natural-looking and long-term results, he prefers to use an innovative signature technique that focuses on deep plane facelift (beneath the SMAS, Superficial Muscular Aponeurotic System).
The benefits of this lift include a natural rejuvenation the face by restoring soft tissues to their youthful position and avoids stretching of the skin under tension, the traditional method of performing facelift surgery.
Deep plane face lifting also is associated with faster healing as there is less disruption of the natural anatomy.
Dr De Silva is known for natural-looking results and uses specialised techniques that foster a natural and conservative rejuvenation.
Sedative anaesthesia, also known as “twilight anaesthesia,” is a combination of local and sedative medications.
At the centre, Dr De Silva has pioneered our own advanced form of twilight anaesthesia, based on practices in the USA. This specialised sedation uses tiny amounts of four to five sedation medications that cumulatively give a relaxing and safe experience to avoid the risks and side effects of general anaesthesia.
A much shorter recovery period than general anaesthesia, with most patients able to leave 30 to 60 minutes following the procedure. These are the most advanced sedation techniques available, and the optimal option for patients considering facelift surgery.

Having no bruising is a distinct advantage after facelift surgery, as in men, it is difficult to camouflage, most women can use concealer to cover bruising.
Using Dr De Silva’s techniques, only 10-20% of patients have bruising after surgery. Dr De Silva uses the latest in technology to enhance recovery after surgery. Utilising our own growth factors, and advanced techniques used by professional athletes, your recovery can be speeded up.
Many men are concerned they will look too different after surgery, and family or friends will notice. The nose is a key element of facial appearance as it lies in the centre of your face.
Good male surgery is discrete, patient’s report to Dr De Silva every week that most of their friends and family could not tell they had undergone surgery until shown before and after photos.
Everyone is familiar with famous male actors and musicians who look like they have had too much work done. The Key is men, “less is more”, from Dr De Silva’s experience most men do not want a “done” appearance and must look natural.

This man in his 60s had found he was looking increasingly tired and was looking to freshen up his appearance, particularly his eyelids, sagging jowl and neckline. He had never undergone previous cosmetic surgery and was concerned about looking too different or ‘plastic’.
He looked for a surgeon specialising in facial cosmetic and plastic surgery. At his consultation, Dr De Silva discussed with him the benefits and downsides of surgery, the recovery period and expectations from surgery.
The skin along his jaw had begun to sag with time, leaving his jaw looking uneven and giving him a web of excess skin below his chin.
In men, the neck and jawline are often more important than the lower part of the face as it gives to a natural result, and for this patient, he and his surgeon decided that a combination with eyelid surgery would give the best result.
During the surgery, Dr De Silva made carefully make incisions and then lifted the tissue and muscles under the skin.
The after photographs show a more rejuvenated appearance with fresher-looking eyes, face and neck. His recovery was relatively quick, with minimal discomfort. The patient was very pleased with the results of his surgery.
Dr De Silva frequently combines rhinoplasty with other facial procedures:
Rhinoplasty is regarded as a challenging aspect of facial plastic surgery as the degree of detail is fine. Over 30% of all Dr De Silva’s patients have undergone previous surgery and are termed revision procedures.
