A mini facelift (Rhytidectomy) is a cosmetic procedure that rejuvenates early signs of facial ageing, characteristically it involves less surgery, is less invasive and has a faster recovery. The surgery is most beneficial to younger patients in their 40s to 50s who have some early sagging of the soft tissues of the lower third of their face, including jowls and deeper nasolabial folds with relatively good skin elasticity.
What are the indications for a Mini-Facelift?
Throughout one’s life, the sun’s radiation, stress, gravity, and other factors cause the skin to sag and fit more loosely. Areas of the lower face and neck, in particular around the jaw line, are especially prone to the ageing process. Characteristically soft tissues of the face can hang over the jaw resulting in jowls creating an irregular jaw line. In addition descent of the facial tissues results in increased Though these changes are gradual, wrinkles and other signs of ageing can make individuals “feel older” than they really are.
What is different about a Mini-Facelift?
Dr. De Silva’s technique for mini-facelift uses small incision compared to a conventional facelift, this results in a shorter operation, less resources and faster recovery. The incision is in front of the ear is hidden in a natural crease in the skin and along the tragus of the ear. The surgery lifts the soft tissues of the neck in a natural vector restoring them to the original location leaving a natural looking appearance. Meticulous closure of the skin is required to avoid a stretched unnatural look. In some people there can be a moderate amount of skin laxity, these patients require an extended incision behind the ear to give a smooth and natural result.
Surgery for a facelift involves the contraction and realignment of facial and neck skin, and in some instances the removal of excess fat deposits. Dr. De Silva will sometimes combine a mini facelift with other facial procedures to enhance the result. A common sign of facial ageing is loss of facial volume and fat stem-cell transfer at the same time as a mini-facelift enhances the result by rejuvenation of the loss of facial volume.
Clinical signs that indicate benefits of a Mini-Facelift
- Younger patients with early facial ageing
- Sagging skin, soft tissues, and fat in the face and neck
- Excess skin around the neck
- Sagging jaw line with jowls
- Deeper crease lines along the nose, mouth, and chin
What does a Mini-Facelift involve?
The incisions for a mini facelift begin in the temporal hair area and are extended downward, in a natural crease in the skin, behind the tragus of the ear and then around the ear to the earlobe. With patients with additional skin laxity, Dr. De Silva will sometimes extend the incision behind the ear to smoothen out the skin contours and enhance the natural looking result. In male patients Dr. De Silva choses a natural skin incision anterior to the tragus to preserve the natural hair growth in the beard area.
Immediately after surgery, the face will have support bandages to decrease the recovery time and to reduce swelling. Post-operative instructions include the use of ice for ten minutes every hour for the first three days and rest in order to speed up the healing and recovery process. Dr. De Silva keeps a removes most stitches within a week. Patients may have some minor pain associated with surgery, and all patients are given oral medication which is controls this. While complications are rare, patients can minimize potential problems by carefully following the directions given after the procedure.
Patients should expect some bruising and swelling after surgery and should allow a minimum of 2-weeks off social activities, approximately 80% of the swelling resolves over the first week. Most patients return to work about ten days to two weeks after surgery. It is important to avoid strenuous activity, for approximately 4-weeks after surgery.

Different facial shapes, ethnicity and individual anatomical changes require different types of facelift, customized to the individual patient to achieve a natural looking and long lasting results.
Ageing of the face is characterised by three principal changes, loss of volume of the fat and soft tissues in the face, gravitational descent of facial soft tissues and laxity of the skin. Key to natural looking results is an artistic and anatomical understanding of these three elements so the elements of the facial ageing can be both restored to their previous appearance.
Mini Facelift surgery requires lifting of the soft tissues of the face that are often characterised by jowls, irregular jaw line and deep lines in the lower third of the face. Often elevation of the soft tissues of the face results in an excess of skin that requires removal. Dr. De Silva hides such incision in natural creases in the skin and in the inner tragus of the ear to avoid visible scars while giving the best possible results. Elevation of the skin alone in older techniques leaves stretched scars that are the hallmarks of surgery, Dr. De Silva takes meticulous attention to elevating the tissues beneath the skin to enable scars to heal better free of tension. Dr. De Silva presented internationally on Advanced Neck Lifting Techniques at the 43rd Annual ASOPRS Scientific Symposium in Chicago, IL, USA.
