The Perfect JawAugust 5, 2015
As with other facial features, there are different characteristics of an ideal jawline for men and women. The ideal jaw for a man is strong, with more prominent angles and a pronounced chin. The latest trend has been stronger, squarer jawlines than those of the past, and some men even choose a chin augmentation to balance out their naturally strong noses in lieu of a rhinoplasty. For women, the ideal jaw has softer, more feminine characteristics, though of course each patient will have a different definition of “ideal,” depending on their personal facial characteristics.
A well-defined, proportional jawline and chin are characteristics of youth and attractiveness, and these factors can even improve your performance in the workplace. There are now many options, both surgical and non-surgical, to provide the desired facial changes. A great non-surgical option is a chin augmentation using fillers such as Radiesse and Voluma. A filler chin augmentation is not a permanent option, as the fillers will naturally be broken down and absorbed by the body within 6-12 months, but it is a great option for those who are not yet ready for surgery but would like to see what the changes would look like.
For patients desiring a permanent solution, there is the option of a surgical chin augmentation, using a medical grade silicone chin implant. Dr. De Silva tailors the implant to each individual, and offers great flexibility in both size and shape when determining what changes would best fit their face. Men and women need different sculpting of the implant to give the best natural looking result, men a stronger more square jawline and for women a more ‘heart-shaped’ jaw line.
To achieve their desired results, some patients may even benefit from a jaw advancement surgery in conjunction with their chin augmentation. Though this surgery is more invasive, it can help correct a recessed chin or jawline and bring it into more prominence. With time everything has a tendency to sag as a result of laxity of soft tissue, gravity and general ageing, Dr. De Silva ensures that the chin implant stays in its ideal position long-term by securing the implant with two tiny titanium screws, these ensure that no movement can occur with time.
For those who already have a great underlying structure, but whose chin and jawlines have become less defined over the years due to the aging process, face and neck lifts can help improve skin laxity. By addressing the sagging skin, the youthful contours are once more revealed, and can be a great way to look and feel younger and rejuvenated.
If you are interested in learning more about improving your facial balance by addressing the jawline, please contact Dr. Julian De Silva for a consultation. Dr. De Silva is experienced with chin augmentations and can help determine the best solution for you.
Fillers: How Much Is Too MuchJuly 20, 2015
Dark circles under the eyes, deep “smile lines,” droopy nose, and sunken cheeks are some of the most apparent signs of aging that appear on the face as we get older. However, in recent years, technological advancements in the cosmetic field have allowed millions of Americans every year to flock to cosmetic surgeons in order to turn back time and restore their youthfulness. According to the American Society of Plastic Surgeons (ASPS), a total of 15.6 million cosmetic procedures, both surgical and non-surgical, were performed last year. A large percentage of these procedures included the use of injectable fillers.
Fillers are substances which can be injected directly through the skin using a syringe. They are very versatile and can be used for various purposes. Some common uses include fixing scars, filling up wrinkles and creases, enhancing cheeks and lips structure, and improving the contours of the face.
- Restylane: This filler is most effective for increasing the plumpness of the face and bringing more volume to the skin. Deep wrinkles and skin folds can be easily reduced by Restylane fillers. On average, this filler will maintain its effects for six months and may even last for up to nine months.
- Juvéderm: Another popular choice of filler is Juvéderm. This works great for lip augmentation, reducing lines around the lips, treating acne scars, and minimizing “smile lines.” If done properly, Juvéderm should last for six months.
- Voluma: A newer generation of filler that has the advantage of a cross-linking structure, this enables the filler to last for considerably longer, 1-year or more. This filler works well for volume augmentation in specific parts of the face.
- Botox: As the most well-known filler available for cosmetic procedures, Botox is the ideal choice for you if you are looking to get rid of crow’s feet, wrinkles on the forehead, frown lines, or to treat muscular spasms on the face. Typically, Botox will maintain its results anywhere from three to four months.
- Dysport: Dysport is a relatively new filler which is very similar to Botox. It relaxes muscles that cause wrinkles on the face. Thus, it is a good option for treating wrinkles around the eyes, lips and on the forehead. When compared to Botox, Dysport is less expensive, as well.
Regardless of which injectable filler you use, inserting too much filler or improper treatment can cause severe side effects, such as malformed and irregular features, excess swelling and bruises, or weakened muscles. Therefore, it is crucial that you only have an experienced surgeon perform the procedures to ensure your safety and to achieve impeccable results.
Some of the fillers, like Botox, have been used by doctors for more than a decade. And many of them are approved by the Food and Drug Administration (FDA). These are all strong proofs that these things work well and can genuinely give you a more attractive overall look.
Dr. De Silva is proficient in the use of fillers and specializes in facial cosmetic and plastic surgeries. Unlike other surgeons who work on various parts of the body (and sometimes fail to produce desired results), Dr. De Silva’s exclusive focus on facial surgeries has enabled him to become exceptionally innovative and highly experienced in his work. Moreover, he goes above and beyond his work to show extra care and concern for his patients.
To him, his patients will always come first. This is evident from his philosophy because he chooses to only perform facial procedures when he is certain that he can make a positive difference in his patient’s life. He will not conduct any procedure if he deems it to be harmful to his patients. So, when you visit Dr. De Silva, you can rest assure you are in safe hands!
Do Non-Surgical Necklifts Really Work?May 8, 2015
Increasingly, individuals seeking to rejuvenate their appearance have been inquiring about minimally invasive rejuvenation therapies, a set of techniques collectively known as a “non-surgical facelift.” Lauded as a way to reap the benefits of cosmetic work without having to endure the recovery time associated with surgery, the non-surgical facelift is an option for those who want a visible boost, but are not ready for a more extensive procedure.
What Is A Non-Surgical Facelift?
A non-surgical facelift can include volume enhancers, smoothing, resurfacing and tightening treatments. To combat volume loss and skin laxity, dermal fillers, such as Juvaderm, Sculptra, Restylane and Radiesse, are commonly injected into sunken areas of the face–often the cheeks–to restore youthful plumpness. To smooth out lines and deep creases, a wrinkle thinning agent such as Botulinum toxin, also known as botox, can be used to soften the formation of wrinkles, this be injected into the skin. In the UK this treatment requires a medical prescription and should be only administered by trained doctors or nurse subscribers. Botox temporarily relaxes muscle movement, thereby smoothing wrinkles associated with facial expressions. Laser resurfacing can prompt collagen production and tighten the skin.
One great advantage of many minimally and non-invasive techniques is that they are typically painless, can be performed quickly and can yield results can be immediately visible. A treatment can be done on a lunch hour and the patient can resume normal activities without a break in their daily routine. With no incisions and sutures involved in the procedures, there is no drawn out recovery time or need for post-surgical pain management. Dr. De Silva utilizes non-needle techniques with the latest technology to minimize the chance of bruising from these treatments compared to traditional treatments. Without bruising most patients there can be no signs a patient has undergone treatment.
So how does the non-surgical facelift stack up against a traditional facelift? Both approaches have their pros and cons. Non-surgical facelift treatments can be highly effective on skin that shows early signs of aging. Therefore, the best candidate is generally younger and has skin that still has some elasticity. A non-surgical facelift cannot counteract note able skin laxity and more advanced signs of aging. While the results of a non-surgical facelift can be long-lasting, they will not be as enduring as a traditional surgical facelift. Typically non-surgical techniques can last from 6 months to over a year, depending on the type of treatment. Though a surgical procedure involves more time and carries some risks, the positive outcome of a facelift can last for many years.
While a non-surgical facelift will not address severe sagging in the face and neck and jowls, the techniques can be transformative on patients who younger, particularly those in their 30s and 40s. For those who would like to avoid surgery and downtime, the non-surgical facelift is an excellent option. Not all the treatments used in non-surgical facelifts are appropriate for every individual, so it is essential to have a comprehensive consultation so that the results can be optimized.
Your Non-Surgical Facelift Consultation
Because the non-surgical facelift is a single therapy, your skin needs to be evaluated in order to identify the combination of treatments that will be most effective for you. The aim is to achieve natural-looking results in the most efficient manner. A customized plan will be recommended based on the overall condition of your skin, the areas of concern, and other factors specific to your case.
More Celebrity Plastic Surgery or Strategic Makeup?March 5, 2015
Uma Thurman’s recent red carpet appearance drew a lot of attention from the press for her “drastic change in appearance.” Many media outlets have speculated on plastic surgery she may have undergone, recalling the plastic surgery speculation furor that happened after Renee Zellweger’s recent changed appearance as well. Many of my patients are now asking – has Uma really had work done?
Uma Thurman Before and After
Though Uma’s new look is very different from her previous red carpet styling, these differences are more due to makeup and styling than plastic surgery. Her eyebrows have been styled much straighter and fuller than before, which makes her look younger. Her hair is slicked back, which allows her features to stand out more than her previous loose face framing locks. Her makeup artist also used more contouring techniques to highlight her cheekbones.
Through these cosmetic changes, we can still see that the fundamental facial structure has stayed consistent. Her eyelid spacing has not changed, which was one of the big indicators with Renee Zellweger’s rumored blepharoplasty. Uma’s red lips may draw more attention, but they are still the same size and shape as before, and her nose has stayed the same as well. Her skin texture is very smooth and luminous, but this is not a drastic departure from a picture taken of her a year previously, and the makeup artist responsible for her look has explained the steps he took to create such a dewy effect.
The Effect of Makeup
Uma’s pictures really highlight how important a tool makeup is in changing a person’s look. By minimizing eye makeup and highlighting the skin and lips, Uma’s makeup artist managed to create a look that caused plastic surgery speculation frenzy. Mascara and eyeliner in particular can greatly change the shape and look of a person’s eyes.
Uma Thurman’s Look Will Not Work For Everyone
Uma’s look was much more high fashion than previous, and it is important to note that this type of look will not suit everyone. Uma has very bright, stand-out eyes that pop even without eye makeup, whereas most people tend to emphasize their eyes to make them look bigger or change their shape with makeup. Her skin is also smooth and wrinkle-free; this allowed her makeup artist to focus on strategies that would provide a youthful, radiant effect. Rather than a matte foundation for the face, he used a glowing foundation layered with a dewy mist to provide a luminous effect. This would not work on older people with wrinkly skin, as the makeup would settle into the folds more visibly.

Liquid Facelift vs. Traditional Facelift- Which One Should You Get?December 16, 2014
As a leading plastic surgeon practicing in the UK, Dr. De Silva is proud to offer his patients an extensive range of procedures. He understands that having work done is a big decision, and that no two patients’ conditions are exactly the same. When deciding between a liquid and traditional facelift, for example, Dr. De Silva makes sure that his patients have a clear understanding as to exactly what each treatment entails, so as to make a more informed decision when choosing which route to take. Both treatments are designed to target facial wrinkling and are directed at tightening the overall look of the face to achieve a younger, rejuvenated look, but each takes a different approach to achieving that goal.
One of the main differences between the two options is that a liquid facelift is a non-surgical alternative to the traditional facelift procedure. While a traditional facelift requires an incision, the use of either general or local anesthesia, and involves a more intensive recovery period, the results of a liquid facelift are nearly immediate and require no downtime. A liquid facelift, as performed in our office, is a broad term that generally includes injectable fillers, like wrinkle reducing treatments and Juvederm. The term can also include the PRP facelift, a treatment in which platelets from your own blood are extracted and transported to the face in order to revitalize and refresh your complexion. A liquid facelift will leave no signs of scarring, can visibly restore lost facial volume, and is a good option for those who are hoping to achieve a younger, more refreshed look but who are afraid of or otherwise unwilling to undergo surgery. Whereas a traditional facelift can take up to two hours to perform and requires you to take about a week off from work, your liquid facelift will be complete in 15 minutes. You will be in and out of the office without having to sacrifice your daily routine to devote to a proper recovery and can literally have the treatment done during a lunch break from work.
Still, this shorter process is directly reflected in the results that the respective procedures yield. Perhaps the most important difference when evaluating the liquid vs. traditional facelift is that the results of the traditional facelift are more dramatic and long-lasting. A traditional facelift has the ability to make you look up to 15 years younger, and is generally sought after by relatively older patients whose skin has loosened as a result of aging, exposure to the sun, or the body’s natural decrease in the skin’s collagen supply. During a traditional facelift, Dr. De Silva will essentially “lift” the saggy skin that has accumulated around your face. This approach is meant as a solution for a more pronounced condition, where the patient demonstrates considerable skin laxity and looseness. As expected, a surgery will yield much more dramatic results than that of a liquid facelift and since underlying muscles are put back to their original location and excess skin is removed, patients will look younger than their age even 10 or 20 years after their facelift surgery. Traditional facelift is recommended for patients who are between the ages of 45-70, while the liquid facelift is more popular among younger patients who are not experiencing as much wrinkling, sagging, and skin laxity.
Dr. De Silva is experienced with variations of facelift surgery, such as neck lift that targets double chin, fat and wrinkles in the neck area and along the jawline; mid facelift that mainly targets the middle facial area around the cheeks; mini facelift for younger patients with smaller scar; fat stem cells facelift to augment surgical results with rejuvenating stem cells; and other facelift techniques. He will advise you as to the best facelift surgery for you as part of your consultation.
Dr. De Silva will more thoroughly explain the pros and cons of both options upon your initial consultation visit so that you can choose the route that works best for your specific needs. As always, Dr. De Silva is devoted to giving you the most natural results so that you can feel confident that your treatment does not look overdone or fake. Whichever route you choose to pursue, liquid or traditional, you can rest assured that you are under the care of one of the most trusted surgeons in London and that your new face will look considerably less aged and tired.
