Best Anaesthesia for Eyelid Surgery

March 20, 2017

Blepharoplasty, also known as eyelid surgery or an eyelid lift, is one of the most popular procedures sought after by our patients. Blepharoplasty expert, Dr. De Silva performs eyelid surgery as a way to correct and refresh patients’ appearance with a natural appearance. After patients undergo this eyelid procedure, they are able to look more refresshed and feel better. However, no matter how qualified your surgeon is, it’s vital to understand the best anaesthesia to suit your individual needs.

Facts about Anaesthesia

Anaesthesia is commonly utilized to aid comfort for individuals undergoing many forms of surgery. There are broadly speaking three types. Local anaesthesia means you are completely awake and only the area being treated is numbed. General anaesthesia means you are completely unconscious and machines are looking after your breathing. For blepharoplasty, Dr. De Silva prefers the use of sedation anaesthesia, also known as twilight sedation, as this has a fast recovery and low downtime with the advantages you literally sleep during your surgery. Dr. De Silva discusses the various forms of anaesthetics with you to determine the best method for your specific procedure.

Local vs General Anaesthesia

General Anaesthesia is reliant on machines to regulate breathing, although very safe, tends to have several hours of recovery, often with feelings of nausea or sickness. Under local anaesthetic with sedation you will be comfortable and the doctor will be able to effectively refresh your eyelids, remove loose skin and fat deposits and re-contour your eyes. Dr. De Silva prefers the utilization of sedation anaesthesia when performing eyelid surgery because the numbing agent provides comfort to the patient and cooperation during treatment. As twilight sedation means you sleep during the procedure when you recovery it is fast, generally over 30-minutes and all patients can go home on the same day of the procedure.

Expected Pain / Dangerous?

Dr. De Silva specializes in facial reconstructive surgery and blepharoplasty procedures. He focuses on safe and natural looking procedures to beautifully transform your features to obtain a more natural and youthful appearance. During the procedure you will sleep during the procedure, by definition you will be breathing yourself yet completely numb and feel no discomfort.

Health Benefits of Blepharoplasty

February 20, 2017

Blepharoplasty, more commonly referred to as Eyelid Surgery, provides individuals with a facial rejuvenation by freshening up the appearance of your eyes and in some people can improve vision. Loose skin and fullness around the upper eyelids can fold over the upper eyelid that can affect vision and interfere with work-related activities. Eyelid surgery often requires removing loose skin and lifts the weight from the upper eyelid, improving both the appearance of the eyelids and vision. Blepharoplasty is a procedure which extracts excess skin by removing and repositioning soft tissue, fat and muscle to more naturally suit your face and correct vision.

This procedure is one of the most highly respected plastic surgeries and has benefited many patients in the United Kingdom. Dr. Julian De Silva provides a customized approach based on the patient’s unique facial composure and proudly utilizes all of the latest technology to provide natural results. His cutting-edge procedure is not only rejuvenating appearances, but providing a host of health benefits as well. The highly regarded cosmetic surgeon is proud to feature a one to two week recovery period from eye lift surgery.

Four benefits of undergoing Blepharoplasty

  • Rejuvenated appearance: Guess what causes bags and puffiness can wreak havoc underneath your eyes? The sullen appearance may be attributed to lack of sleep, but the real culprit is actually excess skin and fat coming forwards as a bulge, all eyelid surgery needs to be tailored to the individual person. Blepharoplasty reduces skin, resulting in a more youthful and refreshed appearance. Although many patients try creams and potions to try and improve this appearance, often the changes require blepharoplasty to make a true difference.
  • Reducing fine lines: During eyelid surgery, a remarkable thing happens. The skin surrounding your eyes is lifted, which reduces the amount of lines, wrinkles and creases evident on your face. Dr. De Silva specializes in the art of facial cosmetic surgery and proudly incorporates advanced techniques into each treatment.
  • Headaches related to vision: In certain circumstances, the excess skin can weigh heavily on the upper eyelids resulting in headaches. In some patients removing the loose skin and opening up the eyelids can relieve the weight on the eyelids and treat the cause of the headaches.
  • Improved vision: Did you know drooping upper eyelid skin can effect the edges of the vision. Excess soft tissue can reduce visibility, Dr. De Silva’s techniques assisted procedure safely improves vision with this revolutionary procedure.

It is imperative to note cosmetic surgery can provide you with impressive results but isn’t the solution for major medical concerns. If you have a significant ophthalmological condition including diabetes, glaucoma or dry eyes it is important you notify your plastic surgeon before undergoing surgery so necessary precautions can be taken which for some patients may required additional safety tests.

Why choose Dr. De Silva

Dr. Julian De Silva, MD is an experienced facial plastic surgeon based out of London and is considered one of the United Kingdom’s best plastic surgeons with a specialty in eyes, nose face and neck areas. Tanning After Facial Plastic Surgery

May 26, 2015

19422504_s

Perfect Season for Facial Plastic Surgery?

If you have decided to undergo plastic surgery and are ready to line up your appointment, you may want to consider scheduling the procedure for the fall or winter months. There are several compelling reasons to book a fall or winter visit to the cosmetic surgeon: cold weather can be an excellent excuse to nest and take the time needed to recuperate; the holidays can be a convenient opportunity to score an extra day or two off work; and, perhaps most importantly, autumn and winter skies can offer some buffer to excessive sun exposure.

After a facial plastic surgery procedure, you can help your body heal by moderating activity, following a healthy lifestyle, and minimizing UV exposure. It is best to slow down, drink plenty of water, and treat yourself to some rest and relaxation. Tanning–best eschewed under normal circumstances–should absolutely be avoided for up to six weeks following any facial plastic surgery such as a blepharoplasty, a rhinoplasty, or a rhytidectomy. As most of us already know, UV light damages the skin and accelerates aging. Post-surgical skin is particularly sensitive to UV exposure because its natural defenses are down. Not only is tanning an impediment to the healing process, it increases the risk of undesired side effects, including permanent skin discoloration. If you tan or burn, you definitely up the chances of creating visible scars. Unfortunately once a healing scar has been burned, the damage is done. It is best to err on the side of prevention. So, be sure to carefully cover any incisions sites.

Recovering from Plastic Surgery in a Sun-Drenched Place

Although the autumn and winter months may be more conducive to healing in some climates, a season-based appointment may not make a difference if you live somewhere with year-round sun. If you are accustomed to a daily dose of sunlight, you will have to be extra vigilant in protecting your skin after facial plastic surgery. Wear a hat, stay in the shade as much as possible, and always wear a high SPF sunscreen. Bypass that trip to the beach or the pool and instead enjoy a picnic in the leafy shadows of a tree.

Although it may be tempting to bask the face in sunlight once your skin has fully healed, you may want to rein in that urge. In truth, protecting the skin from UV rays should be part of everyday life. To increase the longevity of your facial plastic surgery, it is highly recommended that you make these post-procedure sun habits a part of your normal routine. Whether you undergo a blepharoplasty, rhinoplasty, or facelift, the same rule applies: always safeguard your skin.

More Celebrity Plastic Surgery or Strategic Makeup?

March 5, 2015

Uma Thurman’s recent red carpet appearance drew a lot of attention from the press for her “drastic change in appearance.” Many media outlets have speculated on plastic surgery she may have undergone, recalling the plastic surgery speculation furor that happened after Renee Zellweger’s recent changed appearance as well. Many of my patients are now asking – has Uma really had work done?

Though Uma’s new look is very different from her previous red carpet styling, these differences are more due to makeup and styling than plastic surgery. Her eyebrows have been styled much straighter and fuller than before, which makes her look younger. Her hair is slicked back, which allows her features to stand out more than her previous loose face framing locks. Her makeup artist also used more contouring techniques to highlight her cheekbones.

Through these cosmetic changes, we can still see that the fundamental facial structure has stayed consistent. Her eyelid spacing has not changed, which was one of the big indicators with Renee Zellweger’s rumored blepharoplasty.  Uma’s red lips may draw more attention, but they are still the same size and shape as before, and her nose has stayed the same as well. Her skin texture is very smooth and luminous, but this is not a drastic departure from a picture taken of her a year previously, and the makeup artist responsible for her look has explained the steps he took to create such a dewy effect.

The Effect of Makeup

Uma’s pictures really highlight how important a tool makeup is in changing a person’s look. By minimizing eye makeup and highlighting the skin and lips, Uma’s makeup artist managed to create a look that caused plastic surgery speculation frenzy.  Mascara and eyeliner in particular can greatly change the shape and look of a person’s eyes.

Uma Thurman’s Look Will Not Work For Everyone

Uma’s look was much more high fashion than previous, and it is important to note that this type of look will not suit everyone. Uma has very bright, stand-out eyes that pop even without eye makeup, whereas most people tend to emphasize their eyes to make them look bigger or change their shape with makeup. Her skin is also smooth and wrinkle-free; this allowed her makeup artist to focus on strategies that would provide a youthful, radiant effect. Rather than a matte foundation for the face, he used a glowing foundation layered with a dewy mist to provide a luminous effect. This would not work on older people with wrinkly skin, as the makeup would settle into the folds more visibly.

Non-Surgical Procedures For A Rejuvenated Look

More mature individuals who wish to look radiant without going under the knife can consider some non-surgical alternatives. Dr. Julian De Silva is a London-based facial cosmetic surgeon. His specialty is the face, neck, eyes, and nose. There are several non-surgical procedures that Dr. De Silva recommends to improve the skin tone and reduce the effects of aging. Platelet-Rich-Plasma therapy (also known as PRP) has been proven to improve skin quality and improve healing after surgery or laser treatments. Laser Resurfacing has also grown in popularity as people are taking advantage of advanced laser technologies to achieve smoother, younger-looking skin. Dr. De Silva has extensive experience and expertise with these technologies and ensures that his patients always have incredible, natural-looking results that look like a refreshed version of themselves. Why Do Some Celebrities Look Different After Plastic Surgery?

January 14, 2015

Celebrity Plastic Surgery – The Drastic Changes

This is a very topical question; we have all seen celebrities who appear to look very different after undergoing plastic surgery. One of the most common surgeries that can truly alter a person’s appearance is a blepharoplasty or upper eyelid surgery. Good examples of this are Renée Zellweger and Mickey Rourke.

Renée Zellweger Before & After

Renee_Zellweger_Eyelid_Surgery

When Renée was younger she had hooded eyelids, relatively low eyebrows and her upper eyelid could barely be seen. Hooded eyelids can be genetic or acquired with age. Some examples of young celebrities with hooded eyelids are Blake Lively, Jennifer Lawrence and Taylor Swift. Renée  Zellweger is one example of a person born with hooded eyes that became even more hooded with age. The effort to restore a youthful look to her eyes was overachieved, as the surgeon overcorrected Renée ’s eyelids, causing them to lose their distinctive hooded feature.

In more recent photographs of Renee, the difference in her upper eyelid can be seen. The eyes look more open and significantly different from the distinctive hooded eyes of her youth.

Mickey Rourke Before & After

Mickey_Rourke_Eyelids

Micky Rourke looks very different than he did when he was younger and it is particularly his eyelids that have changed with time and alleged plastic surgery. When we look at people we naturally look at the eyes in communication and changes in this area can make a celebrity look very different if the surgery performed is not conservative and specific measures are not taken.

Mickey had hooded upper eyelids when he was younger. This is a common appearance in men which often makes men’s eyes appear smaller. In more recent photos of Mickey, more of his upper eyelid can be seen and as a result, his eyes appear larger. Having more visible eyelids is common in many men; many other A-list celebrities including George Clooney, Richard Gere and Tom Cruise all have relatively full eyelids and low eyebrows. Eyelid surgery in men needs to take these factors into account.

Dr. De Silva specializes in blepharoplasty and eyelid lift surgery, taking these aspects into account is of key importance in giving a natural looking rejuvenation. With good blepharoplasty surgery, a person will look the same after surgery, only refreshed.

Why Has This Happened?

Eyelid surgery, also known as blepharoplasty or eyelid lift, can be achallenging cosmetic surgery as it can change a person’s appearance, though when the surgery is performed to a high standard the surgery looks completely natural.
If too much soft tissue is removed, or if the natural shape and creases of the eyelid are not considered or respected, eyelid surgery can completely change a person’s appearance. All of us look at each other’s eyelids in communication whether socially or professionally, and a change in the appearance around the eyelids can really change a person’s appearance.

The results from blepharoplasty are dependent on both technical excellence and artistry by the surgeon. As a specialist in facial cosmetic and plastic surgery, Dr. De Silva only performs facial surgery. Blepharoplasty, also known as an eyelid lift, is the most common surgery that he performs. “I have performed surgery for many celebrities, actors, actresses and singers, and no two people need the same surgery. It needs to be tailored to the individual to look natural. It is key with eyelid rejuvenation and blepharoplasty to consider with every celebrity how they looked when they were younger to avoid a change in the person’s appearance and discuss with the individual what they would like to achieve. The surgery can then be tailored to achieve this. I use my own customised techniques to ensure my patients have natural looking results. This does result in the surgery taking more time; however, my philosophy is to do things only once and with a natural looking outcome,” said Dr. De Silva.

Can Plastic Surgery Be Reversed?

The upper eyelid is very intricate in terms of anatomy and function. It is challenging to reverse the previous surgery and to restore more volume to the upper eyelid. More than 20% of all eyelid surgeries Dr. De Silva performs are revision surgeries on patients who have had primary surgery elsewhere. Once volume is taken away from the upper eyelid it can be difficult to restore it while still maintaining a natural look.

Surgery to restore volume to the eyelids often includes a number of specialized techniques including the use of fat transfer, CO2 laser, canthopexy, 3D telescopes and tissue glue. The surgery is highly intricate and specialized as less than one millimeter of a change can make all the difference. For this reason, it is important that patients seek a facial plastic surgeon to perform eyelid surgery and revision eyelid surgery. A general plastic surgeon that performs surgery on the entire body is accustomed to surgery of centimeters of the breast and body. Eyelid surgery requires fine millimeter precision work for a great result.

“The advice I would give anyone looking to have eyelid surgery is to take your time and research your surgeon. Natural surgery requires a degree of artistry by the surgeon to ensure that your appearance is only a rejuvenation of how your eyes used to look, not a complete overhaul,” said Dr. De Silva.

Why Does Renée Zellweger Look So Different?

December 31, 2014

This is a very topical question that many of my patients have asked me. It is difficult not to be curious when you come across photos of Renee and how much her appearance has changed over the past few years. Many articles in the media have suggested she has had some radical plastic surgery, so what has really happened?

Renee_Zellweger_Eyelid_Surgery

What Has Changed?
Renée’s appearance has changed, specifically her eyelids. When she was younger she had very full upper eyelids, relatively low eyebrows and a relatively short distance between her eyebrows and her eyelid. As a result, you could barely see her upper eyelids. They were quite hooded. Now you can see a great deal more of her eye and her upper eyelid.

While most women have relatively high eyebrows and a more open eyelid appearance, it can be an error to over-generalize appearance.

Renee_Zellweger_Before_And_After

Why Has This Happened?

It appears that Renee had an upper eyelid blepharoplasty. It looks as if too much soft tissue was removed with the eyelid surgery, changing her appearance. All of us look at each other’s eyelids in communication whether socially or professionally, and a change in the appearance around your eyelids can really alter person’s appearance.

As a specialist in facial cosmetic and plastic surgery, I only perform facial surgery and blepharoplasty (also known as an eyelid lift.) Eyelid surgery is the most common procedure that I perform. A key factor in eyelid rejuvenation and blepharoplasty is maintaining the patient’s facial identity. A surgeon must consider what the patient looked like when they were younger so as to avoid an unrecognizable change in his/ her countenance.

Eyelid surgery is both delicate and intricate as the human eye can notice even a one millimeter difference between the two eyes. A general plastic surgeon is accustomed to performing surgery of the centimeters of the breast and body. Eyelid surgery requires fine millimeter precision work for a great result. It is important to discuss what the patient would like to achieve; the surgery can then be tailored to achieve this.

I use my own customized techniques to ensure my patients receive natural-looking results. The attention to detail does require a longer surgery time, however my philosophy is to do things only once and with the best outcome.

Can This Appearance Be Changed?

The upper eyelid is very intricate in terms of anatomy and function. It is challenging to reverse the previous surgery and to restore more volume to the upper eyelid. Approximately 20% of all eyelid surgeries I perform are revision surgeries on patients who have had an initial blepharoplasty elsewhere. Once volume is taken away from the upper eyelid it can be difficult to restore the appearance while still maintaining a natural look. Fat transfer to the upper eyelid is a viable option for restoring volume to the upper eyelid, but it is a highly intricate and specialized procedure that requires an expert hand. For more information on eyelid surgery see this web page on eyelid surgery.

Avoid the Situation in the First Place

The advice I would give anyone looking to have eyelid surgery is to take your time, and research both the procedure and your surgeon. Watch my video for suggestions and tips for choosing the best blepharoplasty surgeon for you.

Natural surgery requires a degree of artistry by the surgeon to ensure that your appearance is only a rejuvenation of how your eyes used to look.

Although Renee’s appearance has changed she still looks healthy, rejuvenated and happy.

How to Choose Your Blepharoplasty or Eyelid Surgeon

December 22, 2014

Eyelid Surgery in UK - pic

Choosing the best blepharoplasty surgeon is a common concern for many individuals considering an eyelid lift. My name is Dr. Julian De Silva; I am a specialist in facial cosmetic and plastic surgery. I only perform facial surgery and blepharoplasty is the most common surgery that I perform. At my patient’s request I wrote this blog to help interested blepharoplasty candidates choose the best surgeon for them.

Blepharoplasty, also termed eyelid surgery or eyelid lift, is a popular procedure as it can give a person a fresher and younger appearance with relatively fast recovery.

At the same time very topical in the news are several celebrities that look very different after plastic surgery. Eyelid surgery is a procedure that when completed well looks natural and refreshed, however it can change a person’s appearance. Eyelid surgery is a challenging plastic surgery as every person’s eyes are different and a good result is dependent on both technical excellence and artistry.

To find the best Blepharoplasty surgeon for you does require taking time and researching your surgeon. At the end of the day this is your face and you need to put some time into this. Here are a few aspects to consider:

  • A plastic surgeon that specializes in the entire body is used to surgery of centimeters of the breast and body. Blepharoplasty requires fine millimeter precision work for a great result. Find a surgeon who specializes in facial cosmetic and plastic surgery and who performs cosmetic Blepharoplasty on a weekly basis.
  • Artistic ability of the surgeon to rejuvenate your eyelids in terms of your unique facial characteristics can be evaluated with before and after photographs. Ask to look at ‘before’ and ‘after’ examples of the procedure you’re looking into.
  • Request to be put in touch with past patients. Most surgeons are more than happy to direct potential new patients to past patients to help build confidence and trust. This is a relativity new approach which I strongly encourage! If a surgeon doesn’t want you to speak to any of his/ her former patients, ask yourself why.
  • Look for a surgeon who specializes in Cosmetic Blepharoplasty as opposed to reconstructive surgery. Cosmetic surgery is an art form in which mere millimeters will make the difference; reconstructive surgery places more importance on function and less on the aesthetic result. There are many different types of cosmetic eyelid surgery and a blepharoplasty expert will be experienced in techniques including Upper and Lower Blepharoplasty, Asian Blepharoplasty, Male Blepharoplasty, Revision Blepharoplasty, Combinations with Laser and Fat Transfer, 3D-Telescopic Brow lift, PRP, as well as hidden incision techniques including trans-conjunctival blepharoplasty, fat transposition and canthopexy.
  • More than 20% of the Blepharoplasties Dr. De Silva performs are revision cases from patients who have had surgery elsewhere and outside of the UK, including the UK, North & South American, Asia and Africa. Ask your surgeon what their revision rate is. Every surgeon will have a revision rate, which is the number of revisions they perform on their own work. This should be a relativity low number in comparison to the number of treatments which they carry out.
  • Read reviews and see how recent the reviews are. Check online review sites, read magazines which cover aesthetic treatments and check to see if your surgeon has the right credentials for you. Dr. De Silva recommends looking out for surgeons who are members of British, European and American organisations.
  • Listening and Communication. It is important that your potential surgeon listens to you, takes time to understand what you are looking to achieve, and makes sure you feel rapport with the surgery. Your surgeon should also make himself readily available to follow up with and support you after your surgery.
  • Revision surgery can require more complex surgical techniques; look for an expert who is experienced and specializes in complex revision surgery.

Take your time. Your face is important; do your research and come to your own decision. You are invited to set up a consultation with Dr. De Silva in his London private practice and discuss your needs. Out of town patients often send photos via email and interact with him on Skype prior to face-to-face meeting.
